No Refund Policy
I understand that there are “NO REFUNDS”. If you paid (for any service/program) and your child(ren) and/or you did not participate, or you decide that your student(s) can no longer participate for any reason from the date that you pay, please consider your payment a donation in correlation to the Biblical Principal of “Giving”. We are not able to provide “refunds”. However, we can send you a “tax letter” indicating that you donated to us. The reason for “NO REFUNDS” is that we use every dollar once received, to immediately cover expenses for operations, uniforms, equipment, hardware, etc. I agree to accept this policy.
Parents Code of Ethics
You agree to drop off and pick up your child(ren) on time to practices, events, activities and games? Do you agree to act as a positive role model for youth, encouraging youth leadership skills and leading by example to apply core spiritual values? Do you agree to provide a safe and healthy environment, comfortable settings, nutritional value and protect all youth affiliated and/or indirectly affiliated with the with Prince George’s Bears from internal and/or external hazards?
Subscription Agreement
This subscription fee must be paid even during the off season or if there is a sport that you would like your child to participate in that may not have started or occurred. You can only cancel if you decide not to continue participating. THERE ARE NO REFUNDS OF THIS FEE. This cost is per child and is used across all sports as long as the recurring fee is not cancelled or suspended. This cost ONLY covers administrative fees such as field permits, insurance, equipment cleaning and reconditioning or the purchase of new equipment, operational expenses, marketing and promotion. THIS FEE DOES NOT COVER: the cost of practice gear, uniforms or other miscellaneous items.
Informed Consent, Waiver and Acknowledgment
I hereby give my approval for my child’s participation in any and all activities prepared by the Youth Athlete University, Inc. – DBA PG Bears, Prince George’s Bears or PG County Bears. In exchange for the acceptance of said child’s participation, I assume all risk and hazards incidental to the conduct of the activities, and release, absolve and hold harmless NYAA and all its respective officers, agents, and representatives from any and all liability for injuries to said child arising out of traveling to, participating in, or returning from activities including any transportation that coaches and/or parents, team administrators, staff members, etc. use to transport said child. In case of injury to said child, I hereby waive all claims against NYAA including all coaches and affiliates, all participants, sponsoring agencies, advertisers, and, if applicable, owners and lessors of premises used to conduct the event.
There is a risk of being injured that is inherent in all sports activities. Some of these injuries include, but are not limited to, the risk of fractures, paralysis, or death. I understand and agree to allow my student’s name, photograph, likeness and image, and video/audio to be used in for marketing purposes and communication vehicles for promotion, advertising and any other form of multimedia including documentary’s, online and social media use. This may include, but is not limited to, magazine articles, web features, newspapers, magazines, outdoor properties, and specialized microsites.
I hereby authorize the those acting on its behalf to: Record my student’s image, likeness, and/or voice on a photographic, video, audio, digital, electronic, or any other medium; Use, reproduce, modify, exhibit, and/or distribute any such recording in any medium for any purpose that the company may deem appropriate, including promotional, educational or advertising efforts with no compensation to me; and Use my student’s name in connection with any such recordings or uses.
I understand that I shall have no right to inspect or approve any such recordings and uses and that they shall remain the property of the company. I release the Youth Athlete University, Inc., PG Bears, PG County Bears and any other associated name and those acting pursuant to its authority from liability for any violation of any personal or proprietary right I may have in connection with all such recordings and uses. I have read and fully understand the terms of this release.
